Line 1 — Gross Monthly Income. Gross income means all income from all sources, including salaries, wages, commissions, bonuses, dividends, severance pay, pensions, interest, trust income, annuities, capital gains, Social Security benefits, workers’ compensation, disability insurance, veterans’ benefits, spousal support, rental income, gifts, prizes, or awards.

Do not include: public assistance (TANF, SSI, food stamps, energy assistance, child care subsidies, etc.), child support received, or secondary employment income used solely to pay a child support arrearage.

Always enter as a monthly figure. Use the converter if your income is weekly, biweekly, semimonthly, or annual.

Line 2 — Spousal support adjustments. If a party pays spousal support under an existing court/administrative order or written agreement, enter it as PAID (subtract). If a party receives spousal support under an existing order/agreement, enter it as RECEIVED (add).
Line 3 — Support of other children. Subtract (a) child support paid under an existing order for other children, and (b) for “other children in household,” subtract the amount from the basic schedule using that parent’s income as if it were the total for the number of those children. These deductions are presumptive.
Line 4 — Deductions allowable by law. For self-employment/partnership/closely-held business income, subtract reasonable business expenses. Include one-half of self-employment tax, if applicable.
Line 7.b — Health care coverage. Enter the monthly cost attributable to the child(ren) in this case. If per-child cost isn’t provided, subtract the policy-holder’s individual premium from the total and divide among remaining covered persons. “Reasonable cost” is generally ≤5% of the responsible parent’s gross income.
Line 7.c — Employment-related childcare. Enter the actual or required cost for employment-related childcare (allocate if mixed purposes).
Line 12 — Adjustments. Use the items below only if applicable. Enter amounts for each parent as plus or minus, per the instructions.
Line 12(a) — Disability derivative benefits. If amounts paid to/for the child were included in a parent’s gross income, subtract that same amount from that parent’s obligation.
Line 12(b) — Other adjustment (±). Enter any court-approved/statutory adjustments not captured elsewhere. Use positive to add, negative to subtract.
Line 12(c) — Other adjustment (±). Use only if an additional separate adjustment is needed beyond 12(b). Again, positive adds; negative subtracts.
